Frequently Asked Questions
How do I create an account?
When you fill in either an Airway Labs slip or one of your own and send it with the patient’s models, we automatically create an account for you. Once we receive your case, we will enter you into the computer database. Your account number will appear on your invoice. When we return the appliance to you, we will include our lab slips and a UPS label (for future cases or returns) with the case.
What do I need to send?
Send stone models of the patient’s teeth. The models do not need to be mounted on an articulator. It is very important that the stone models are accurate and without air bubbles. If they are not accurate, it will be more difficult and time-consuming to seat the appliance. There is a space on the lab slip to write special instructions if needed. Important: A bite registration is not needed for the TAP I, however, we do require a 60% protrusive bite registration for the TAP 3.
How should I send the models?
Airway Labs provides pre-paid UPS shipping labels. Please contact our office and a label will be faxed to you. Please wrap the models well so they do not break in transition from your office to Airway Labs. NOTE: Airway Labs is not responsible for broken models when sent from the doctor’s office. It is the doctor’s responsibility to pack the stone models in protective wrap so they will not break in transportation. The appliance cannot be fabricated from broken models.
Where do I send the models?
If you do not wish to use our pre-paid UPS label, please send the case to the following address:
Airway Labs
3418 Midcourt Road
Suite 114
Carrollton, Texas 75006
What is the lab turn-around time?
Once we receive the models from you, there is a five business day lab turn-around. The appliance is shipped back to you via two-day UPS. You can also choose to have the case “rushed” for an additional $75. In the case of a rush, you will receive the appliance back 24 hours from the time the lab receives your case. Please note that Airway Labs is closed on Fridays.
Will the stone models be returned?
Yes. Please be aware that damage may occur during fabrication but every effort is made to preserve the casts’ originality.
What are your office hours?
Airway Labs is open Monday through Thursday from 7:00am to 5:00pm (Central Standard Time). The lab is closed on Fridays.
When is payment due?
An invoice is returned with each case. You may pay from this invoice upon receipt. Also, a statement is sent out at the end of the month. Payment is due 30 days from the invoice date.
When should I schedule the patient to deliver the appliance?
You should allow enough time for the appliance to return before you schedule a patient. Although UPS guarantees their timeliness, it would be best to schedule the patient two or three days after the appliance is expected to arrive. Also, account for the time it will take the case to reach Airway Labs from your office.
